Health Action Progress Tracker:
A 2020 Vision for Sonoma County

In 2007, Health Action was launched as an effort to convene all local stakeholders to improve health and well-being in Sonoma County. Health Action developed a vision and plan for improving community health by addressing key social, economic, and environmental determinants of health. As part of that plan, Health Action identified 2020 goals and targets for tracking community health improvement.
