Health Action : A 2020 Vision for Sonoma County

Health Action is a body of community leaders convened by the Sonoma County Department of Health Services in August 2007 when the local health care delivery system was in crisis: The number of uninsured people in Sonoma County was estimated at 60,000 and growing. The number of physicians and specialists in the county was declining. District hospitals and community health centers were struggling financially, and funding for the health care safety net was deemed inadequate.

Health Action developed a vision and plan for improving community health by 2020 by addressing key social, economic, and environmental determinants of health. As part of that plan, Health Action identified goals and measures to assess progress.

Here, we track how Sonoma County is measuring up to Health Action's desired community health outcomes.
